    To 45th TFS, 2nd Air Division, Ubon Royal Thai Air Force Base, Thailand.

    10 July 1965

    Shot down VPAF MiG-17 with an AIM-7 over North Vietnam. Flight crew at the time of the kill was pilot Capt. Thomas S. Roberts and RIO Capt. Ronald C. Anderson. This kill was the second USAF MiG kill of the Vietnam War. 64-0679 had the callsign of Mink 04 during the mission.

    Transferred to 557th TFS, 12th TFW, Cam Ranh Bay, South Vietnam.

    From By 1977 to 1980

    Transferred to 170th TFS, 183rd TFG, Illinois ANG, Capital Airport, Springfield, IL.

    From 25 September 1980 to 8 June 1986

    Transferred to 123rd FIS, 142nd FIG, Oregon ANG, Portland IAP, OR.
    Operated with markings: U.S. AIR FORCE, OREGON

    August 1988

    To Misawa AB, Japan.
    Assigned as a BDR trainer.

    September 1992

    To National Museum of the United States Air Force Loan Program, Wright Field, Dayton, OH.
