Notes: Taken at Winnipeg as C-FBCI shortly before it was sold to Pen Air in Alaska and became N660PA. It was converted by McKinnon as a G-21C Hybrid (i.e. turboprop) conversion in 1968 from ex-USN JRF-5 msn B-138 (USN BuNo. 87744)

Crashed.
There were two fatalities. The accident occurred at Dutch Harbor, AK, United States. The airframe was destroyed. A search for the airplane was suspended 4 days after the airplane was reported overdue. The search included remote areas of island coastline and mountainous terrain. Low ceilings were reported in the area of departure by the operator. Aircraft damage and injuries are presumed.
