    Constructed as a F-4C-21-MC by at Saint Louis, Missouri, USA.

    First flight.

    Taken on Strength/Charge with the United States Air Force with s/n 63-7680.

    To 497th TFS, 8th TFW, Udorn RTAFB, Thailand.
    Markings added: FP

    By January 1967

    Loaned to 555th TFS, 8th TFW, Udorn RTAFB, Thailand.

    2 January 1967

    During Operation Bolo, this F-4 shot down a MiG-21 over North Vietnam with an AIM-9B. At the time of the kill it was being flown by Col. Robin Olds and 1st Lt C. Clifton (WSO). Callsign was Olds-01.

    By May 1967

    Transferred to 433rd TFS, 8th TFW, Ubon RTAFB, Thailand.

    13 May 1967

    Shot down MiG-17 with AIM-7E over North Vietnam. Flown by Lt Col. F. A. Haeffner and 1st Lt M R. Bever (WSO). Callsign was Harpoon-03.

    By November 1967

    Transferred to 480th TFS, 366th TFW, Da Nang AB, South Vietnam.

    20 November 1967

    Shot down.
    Shot down by AAA Nov 20, 1967 while attacking suspected SAM site. Rear seat ejected safely and was picked up, but pilot not seen to leave the aircraft before water impact. Pilot declared KIA.

    The former USN F-4B 149421 on display at Lackland AFB, TX, is displayed as this aircraft.
