    Taken on Strength/Charge with the United States Army with s/n 67-17500.

    To 355th Aviation Company, 4th Army, Fort Sill, OK.

    June 1968

    Transferred to 176th Helicopter Company, Phu Hiep and Duc Pho, South Vietnam.
    Operated with markings: Smokey 500, 176
    While assigned to the 176th AHC, 17500 was the smoke ship.

    September 1969

    Transferred to 176th Assault Helicopter Company, Phu Hiep and Duc Pho, South Vietnam.

    2 May 1970

    Damaged.
    While smoke covering an LZ for troop disembarkment, UH-1H 67-17500 took 8 hits from enemy small arms fire (7.62MM cailber) on the left side. These hits damaged the electrical, fuel, communication and main rotor systems. 67-17500 was forced to divert the LZ before the area was sufficiently covered in smoke.

    19 December 1970

    Damaged.
    On the morning of 19 December 1970, aircraft 67-17500, a UH-1H departed the 176th AHC for a re-supply mission for the 5th BN, 46th Infantry. The LZ was in an area 5 kilometers west of Ky Tra. UH-1H, 67-17500 had picked up supplies, made one stop, then proceeded to the 03A location for the next stop. The aircraft commander flew over the area and made his high observation for landing suitability. CW2 Rossomme informed the unit to receive the supplies that he did not think the area was very suitable, but he would try to make an approach and make a final evaluation on short final to the LZ. Since there were tall trees bordering the south and east sides of the LZ, CW2 Rossomme made his approach from the west with a crossswind and planned to dogleg his final into the wind. As he started a left turn, 67-17500 began settling with power. CW2 Rossomme immediately increased power and attempted to make a missed approach, continuing his left turn to avoid the tall trees to the east of the LZ. At this time, 67-17500 tail rotor struck a tree, the aircraft spun to the right 180 degrees, causing further damage to the tail section of the aircraft. The aircraft came to rest downslope, upright. CW2 Rossomme completed emergency shutdown procedures as the crew chief and gunner kept the pax on board until the blades stopped turning. There was no fire. There were no injuries and all the crew members were wearing nomex and other required protective gear. All radios were salvaged.

    To U. S. Army Transportation Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX.

    March 1974

    Transferred to Supply Division, 5th Army, Fort Knox, KY.

    April 1974

    To U. S. Army Transportation Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX.

    May 1974

    To Installation Maintenance Flight (INST MAINT FLT), 1st Army, Fort Campbell, KY.
