    Constructed as an AH-1G-BF by Bell at Fort Worth, Texas, USA.

    October 1968

    Taken on Strength/Charge with the United States Army with s/n 67-15769.

    November 1968

    To 388th Transportation Company, RVN.

    December 1968

    Transferred to B Battery, 2-20 Aerial Rocket Artillery, 1st Air Cav., RVN.

    8 February 1969

    Damaged.
    While on an armed visual reconnaissance mission over South Vietnam, AH-1G 67-15769 was attacking enemy targets at 1300 feet and 100 knots when it took 3 hits from Small Arms fire in 7.62MM caliber. The helicopter was hit in the fuel tanks, oil system and weapons pylons. The self sealing fuel tank developed a leak. This helicopter continued in flight and accomplished all mission objectives.

    Transferred to 56th Transportation Company, RVN.

    11 September 1969

    Damaged.
    Damaged on the ground during an enemy rocket attack.

    October 1969

    Transferred to 166th Aviation MT, 34th Group, RVN.

    November 1969

    To Bell Helicopter Company, Fort Worth, TX.
    Depot maintenance and rocket attack repairs.

    May 1970

    Transferred to F Troop, 7/17th CAV, 1st Cav. Division, Chu Lai, RVN.

    30 May 1970

    Shot down.
    During an armed recon mission over South Vietnam, AH-1G 67-15769 was engaging enemy ground targets at 1000 feet and 110 knots when it took 3 hits from an enemy 12.7MM caliber weapon, likely a DShK. The rounds impacted the canopy which caused spalling. One of the crewmen was injured by the impacts. It was diverted after completing some mission objectives. The helicopter made a forced landing near its home base. Aircraft was inspected and found to be in flyable condition and was flown back to base for repairs under its own power.

    Transferred to B Flight, 123rd Aviation Battalion, RVN.

    24 February 1971

    Damaged.
    During an armed recon mission over South Vietnam, AH-1G 67-15769 was attacking a target at 0120 feet and 125 knots when it took a single 7.62MM hit to the fuel systems. The helicopter continued in flight and was able to accomplish all mission objectives.

    Transferred to Supply Division, 5th Army, Fort Hood, TX.

    March 1974

    Transferred to 1st Cav. Division, 5th Army, Fort Hood, TX.

    April 1974

    To Army Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX.

    May 1974

    To Installation Maintenance Flight (INST MAINT FLT), 1st Army, Fort Campbell, KY.

    October 1974

    Transferred to Supply Division, 5th Army, Fort Hood, TX.

    November 1974

    Transferred to 1st Cav. Division, 5th Army, Fort Hood, TX.
