    Summarized NTSB narrative from report number OAK67D0334: There were no fatalities. Incident occurred at 11:25 hours. The damage was substantial. Incident occurred at the Carson City Airport, Carson City, NV. The engine failed or malfunctioned during the initial climb. Landed wheels up while aborting takeoff. There was a partial power loss with the failure of one engine. The emergency circumstances included; forced landing on a seaplane base or heliport...

    Notes: Grumman TBM-3E Avenger (Bu91586) (N9433Z) - Though designed by Grumman, the TBM Avenger was built by General Motors in Trenton, NJ. It was one of the finest torpedo bombers in the Second World War. This example is on display at the Cradle of Aviation Museum on Long Island.
