    30 September 1986

    Crashed.
    The accident occurred at Fognak Island, AK, United States while taking off. The damage was substantial. On takeoff, the right door popped open. The pilot reached across to pull the door closed and lost directional control of the aircraft. The aircraft then struck a tree near the bank of the lake.

    13 July 1989

    Crashed.
    There were two serious injuries and were four minor injuries. The accident occurred at Kodiak, AK, United States while maneuvering. The damage was substantial. The airplane crashed at the 1800 foot level of a mountain pass, near kodiak, Alaska. The airplane was substantially damaged and the pilot and one passenger were seriously injured and four passengers received minor injuries. At the time of the accident the pass was reported closed due to IFR weather conditions.
