    13 December 1970

    Damaged.
    At 0635 hours, Capt. Gary White and his crew departed Bien Hoa AB, in support of the 18th ARVN Division. The mission was Command and Control for a combat assault and events proceeded normally until the accident. An approach to a dirt covered bridge was begun and due to troops being on the bridge, the landing was terminated approximately fifty feet east of the bridge on the roadway. The road was under construction and therefore extremely dusty. The surface was high crowned with both sides approximately twelve inches lower than the center. There was a near vertcal step of twelve inches in height approximately six feet from the center, due to the dust conditions, the aircraft commander decided on a maximum performance take-off to get out of the dusty dondition in the shortest possible time. The take-off was near vertical and at approximately 20 altitude and five to seven knots airspeed, a loud noise was emitted from the engine. Power was lost immediately and Capt. White lowered the collective, pulled aft and right cyclic with right pedal and positioned himself over the roadway. The aircraft landed hard with no forward airspeed. The crew and passengers immediately exited the aircraft and sustained no injuries. The crew remained with the aircraft until it was evacuated.

    To U. S. Army Aviation Systems Command (AVSCOM), Saint Louis, MO.

    June 1971

    To Bell Helicopter Company, Fort Worth, TX.

    January 1972

    Transferred to U. S. Army Aviation School (USAAVNS), Fort Rucker, AL.

    October 1973

    Transferred to U. S. Army Medical Activity, 1st Army, Fort Rucker, AL.
    Likely used for MEDEVAC missions.

    Circa 1986

    Converted to an EH-1H.

    From 1986 to 1988

    Federick Scarfone comments (22 April 2021): Airframe was retro fitted as an EH-1H, and I had the privilege to be the crew chief in the US Army.

    16 August 2007
