    Notes: Photo taken at Phu Bai Airport, Republic of Vietnam (VVPB). The aircraft was assigned to the 20th Tactical Air Support Squadron (TASS). The blue fin markings indicate its detachment to the tactical air control party supporting Military Assistance Command (MACV) Team 3 at Hue RVN.

    There were three fatalities. The accident occurred at Macdill AFB Auxiliary Field (AGR) Avon Park, FL, United States while on appoach.
