    Crashed.
    There were 14 fatalities. The accident occurred at Cusco, Peru. The airframe was destroyed. The foreign authority was the source of this information. S. Accredited Representative to assist the CIAA investigation under the provisions of ICAO Annex 13 as the State of Manufacturer and Design of the helicopter.
